What a DOT visit includes
The examination is performed by a certified medical examiner listed on the FMCSA National Registry, and the result is uploaded to the Registry by the examiner.

On federal forms: we do not host FMCSA forms here. They are revised periodically and an examination completed on a superseded version can be rejected. Download current versions from fmcsa.dot.gov/regulations/medical.
