Before your exam

Your employer has scheduled you for an occupational health exam at MedCare. Read this page once, fill out the packet, and bring it with you. That is all you need to do.

Step one

Download and fill out your packet

Fifteen pages, but you only type your name, date of birth and employer once — it carries through the rest. Set aside about twenty minutes.

Don’t email the completed packet to us. It holds your medical information. Bring the printed pages with you. If you can’t print, come fifteen minutes early and we’ll give you a copy at the clinic.

Step two

Bring these with you

Photo ID

A government-issued photo ID is required. Bring your CDL as well if you have one. Without ID we can’t complete drug testing or a DOT exam.

Your medications

A written list with doses, or the bottles themselves. Include anything prescribed recently, and bring glasses, contacts or hearing aids if you use them.

Doctor’s letters

If you’re treated for diabetes, a heart condition, seizures, sleep apnea, or blood pressure, bring a letter or completed form from that provider. Missing paperwork is the most common reason a clearance gets delayed.

Your completed packet

All fifteen pages, including any you left blank. Don’t leave pages behind — an incomplete packet means a second trip.

Step three

What happens at your visit

Plan for about an hour, depending on which tests your role requires.

AT ARRIVAL

Check in

We take your packet, verify your ID, and confirm which exam your employer has authorized. Come fifteen minutes early if you need to finish paperwork.

DURING

Testing

Depending on your role: physical examination, vision and hearing, breathing test, respirator fit test, and specimen collection for lab work or drug screening.

AFTER

Results

Your work-status determination usually goes to your employer the same day. Anything that depends on an outside laboratory follows that lab’s turnaround.

Your privacy

What your employer does and doesn’t see

Your employer receives a work-status determination — cleared, cleared with restrictions, or not cleared — along with the compliance records the regulations require. Your medical details, history, and the answers on your questionnaires stay in your confidential medical record at the clinic.

Answer the questionnaires honestly. They exist to keep you safe on the job, and what you write is protected.

Questions

Common questions

Can I fill the packet on my phone?
You can open it, but phone browsers often won’t save what you type. Use a computer with Adobe Acrobat Reader, or print the pages and fill them in by hand.
What if I can’t print it?
Come fifteen minutes early and we’ll give you a printed copy at the clinic. Bringing it completed is faster for you, but it isn’t required.
Do I need to fast or stop my medications?
No. Eat normally and take your medications as prescribed. Bring the list with you.
What if I fail part of the exam?
Most issues aren’t disqualifying. Some require a letter from your treating provider or a follow-up test. We’ll tell you exactly what’s needed before you leave.
Who do I call if I have to reschedule?
Call the clinic at 956-233-2103. Let your supervisor know as well — on most programs your employer books the appointment, and some projects are scheduled through a third party rather than by us.
How long is my clearance good for?
It depends on the exam. A DOT certificate runs up to 24 months and can be shorter if a condition requires it. Annual surveillance and respirator fit tests run yearly. Your employer tracks when you are next due.
